Important Health Coverage Tax Documents.
Former employees (or their beneficiaries) who were enrolled in the MERS Healthcare Savings Program and have an account balance may be eligible to receive a 1095-B Health Coverage Form. To request this form, please contact the Human Resources Department. Once your request is received and eligibility is confirmed, the 1095-B form will be provided within 30 days of request.
